NASA: How to Claim Damages from Columbia Debris

Any individual suffering from an injury or damaged property caused by debris from the Space Shuttle Columbia can seek compensation from NASA.

NASA officials said those needing to make claims should file them with one of three agency offices. According to NASA's Office of General Counsel, claims can be made by filling out the Standard 95 form available at the U.S. Department of Justice website. NASA will also provide the forms upon request.
